PATIENT CONSENT TO COLLECT & DISCLOSE INFORMATION

The Privacy Act 1988 requires medical practitioners to obtain consent from their patients to collect, use and disclose that patient’s personal information.

You will need to consent for the Medical Practitioner and staff at WEXFORD NEPHROLOGY to collect use and disclose the personal information relating to you or your child (if minor) as outlined in the Practice Privacy Policy (also available from reception, Privacy Policy on website), which you will have opportunity to read and understand. This consent is active for all correspondence whether by mail, telephone, email or in person.

You understand, that you are entitled to access your health records except where access would be denied as per the Practice Privacy Policy.

You understand that you may withdraw your consent as to use and disclosure of the personal information relating to my child (except when legal obligations must be met).
